Mondeo's have no form of passive regeneration as the exhaust will never get anywhere near the temperature required to burn soot without the extra diesel on the exhaust stroke. It requires 600C+ and even under full throttle you're unlikely to get past 350-400C.
I spoke to someone who owns a fleet of 50 odd cars, 39 of which are ford and most of these are mk4 mondeos. He said that i cant afford to drive with no MAF as the DPF wont regen and said if the MAF is not the issue he was 90% sure it would be the EGR. I have just installed a blanking.plate and reconnected the MAF sensor and taken it for a spin.

The Mk4.5 tend to be 2.0tdci but with 160bhp, as opposed to 2.0 with 140bhp. There are exceptions of course with 2.2's etc The MK4.5 grill is different. Slimmer, two horizontal "bars" as opposed to a larger "grill" The Mk4.5 has a DPF that relies on a ceramic catalyst covering as opposed to a fluid top up - which renders it a "better"(?) system?
